Also an employees personal sandwich was stored on the same shelf. Raw steak was placed above ready to eat carrots and scallions, which can cause cross contamination.
The dishwasher employee did not wash hands in between handling dirty and clean dishes. There were multiple scoops, knives, utensils, and dicers soiled with debris on food contact surfaces.
The rice at Hasaki Grill & Sushi was being held at unsafe temperatures and had to be thrown away. Hasaki Grill & Sushi has had a historical issue with cooling white rice.
